We understand that not one care plan fits all. Daily services can include anything from meal preparation, hygiene, cleaning, and supervision. We will take the time to get to know you and develop an individualized care plan that fits your specific needs.

Companionship is key to a trusted relationship with our caregivers. We not only strive to help you with everyday tasks but want to develop a caring relationship with you. We provide attention and care while helping you to maintain your independence. 


Nursing Care

Our licensed caregivers provide the highest quality of care to both patients and their families. Our goal is to help individuals and their loved ones live happy and fulfilling lives. Whether it be assistance with daily living activities, medication management, or coordination of care, our team will be here for you.



Meals are not only important for nutritional purposes but for socialization.  Our dietary team provide three delicious and nutritious meals in a restaurant style dining environment.  We invite you to join us for lunch when you book a tour.


Who doesn't like to have fun?  We have a new activity calendar full of things to do each month.  Crafts, cooking, exercise, sensory activities...from bingo, to bible study, to bowling, there is something for everyone.  Outside groups are always welcome to come entertain us also.   

Assisted Living vs. Nursing Home

The biggest differences between these two types of senior housing centers revolve around medical services provided, physical plant of each community, and the cost.  

Care and Services

Residents in nursing homes require around the clock care and monitoring.  They typlically live with more complex health care conditions that require the assistance of a skilled nurse or a physical or speech therapist. Some require respiratory care services  

By contrast, residents in an assisted living community generally require custodial care.  It might be a person who lives with memory loss and isn't safe living alone.  Or an adult with mobility problems who needs a little help transferring from their bed to their favorite chair.  

Physical Plant

One obvious difference when you tour both a nursing home and an assisted living community is their appearance.  While a nursing care center generally looks a litle institutional because of the type of care provided, an assisted living usually resembles an upscale apartment community. Nursing home residents are more likely to share a room with another senior.  Residents in an assisted living community usually have thier own apartment or suite and a more home like environment.

Miss Eva is the life of the party!  You can hear her laughter throughout the building.  Everyone she meets is a friend and she genuinely loves her Chisholm Trail family.   

Our Employee of the Month

Juan has amazed everyone with his hard work, awesome skills, and kind heart.  Every task he is given is "no problem"!  You name it, he can fix it and all with a smile on his face!

Lois is an absolute hoot!  We better not say how old she turned on her birthday this month.  Happy birthday sweet lady!
