Built in 1999, Chisholm Trail Assisted Living is the only family owned and operated assisted living center in Duncan, OK. The construction of the building itself was a father & son endeavor between owner/administrator, Dustin Cox, and his father, Rex. Dustin's wife, Dr. Demetra Cox, is the Medical Director for Chisholm Trail Assisted Living. Both Dustin and Demetra are passionate about senior care and the ever changing needs of the elderly.
We understand that not one care plan fits all. Daily services can include anything from meal preparation, hygiene, cleaning, and supervision. We will take the time to get to know you and develop an individualized care plan that fits your specific needs. We specialize in care and daily living assistance to an array of individuals. Compassion, dignity, and quality of life are our priorities. Our care givers will provide an individualized service that you can trust.
Companionship is key to a trusted relationship with our caregivers. We not only strive to help you with everyday tasks but want to develop a caring relationship with you. We provide attention and care while helping you to maintain your independence.

Meals are not only important for nutritional purposes but for socialization as well. Our dietary team provides three delicious and nutritious meals in a restaurant style dining environment. We invite you to join us for lunch when you book a tour.
Who doesn't like to have fun? We have a new activity calendar full of things to do each month. Crafts, cooking, exercise, sensory activities...from bingo, to bible study, to bowling, there is something for everyone. Outside groups are always welcome to come join us also.
The biggest differences between these two types of senior housing centers revolve around medical services provided, physical plant of each community, and the cost.
Residents in nursing homes require around the clock care and monitoring. They typically live with more complex health care conditions that require the assistance of a skilled nurse or physical or speech therapist. Some residents also require respiratory care services.
By contrast, residents in an assisted living community generally require custodial care. It might be a person who lives with memory loss and isn't safe living alone, or someone who just needs assistance with everyday living.
One obvious difference when you tour both a nursing home and an assisted living community is their appearance. While a nursing care center generally looks somewhat institutional because of the type of care provided, an assisted living usually resembles an upscale apartment community. Nursing home residents are more likely to share a room with another senior. Residents in an assisted living community usually have their own apartment or suite and a more home like environment.
There is a monthly additional occupant fee for any apartment type
The 2 bedroom units feature 2 full size bedrooms, each with its own closet, heat and air unit, and vanity. The apartment also features a spacious bathroom, living area and kitchenette with full size refrigerator and microwave.
Medicare does not pay for Assisted Living. The majority of our residents are private pay. Payment is due, in full, at the beginning of the month of occupancy. Assistance is available for filing long term care claims, if applicable.
Our spacious studio apartments come equipped with a microwave, mini-fridge, walk-in showers, and personal heat & air units. Rates are all-inclusive with the exception of telephone service.
Our luxurious suites feature the same amenities as the studio but are larger in size and layout and are equipped with a full sized refrigerator.
